Quick answer: which Spanish stars have had plastic surgery?
- In their own words: Sara Montiel («little things» and a facelift years earlier), Belén Rueda and Marta Sánchez (breast surgery), Ana Obregón (implants and an implant exchange, per Pronto) and Alaska, who talks openly about her operations.
- Non-surgical treatments they made public: Paula Echevarría (brow micropigmentation and ultrasound lifting), Chenoa (radiofrequency) and Bárbara Rey (a gastric balloon for weight loss, which is not cosmetic surgery).
- Denied: Aitana, Concha Velasco, Norma Duval and Elsa Pataky (the nose job).
- Rumours only: Isabel Pantoja’s nose and Malú’s «new face». Neither has confirmed anything.
Spanish actresses plastic surgery stories fill social media after every Goya Awards or Málaga Film Festival red carpet, usually as «before and after» comparisons. But there is a long way between what people say and what really happened. This guide brings together twelve Spanish actresses and singers, from screen legends to today’s pop stars, and separates what they have admitted from what is only rumour.
We also go beyond the gossip. For each name we explain what the procedure really is, how long it lasts, what recovery looks like and what the risks are, with a medical source at the end of each paragraph. If you are considering something similar, you will know what to ask at your consultation. Quotes were given in Spanish; the translations are ours.
Spanish Actresses Plastic Surgery: Breasts, Facelifts and Non-Surgical Treatments
Very few name a procedure, and those who do usually talk about their breasts. This table sums up what each one has said in interviews or on social media, as reported by the outlet cited in her section.

| Star | What she has said | Status |
| Sara Montiel | «Little touch-ups», a facelift years before 1998, laser for skin spots | Confirmed by her |
| Belén Rueda | Breast surgery; says she would never have facial surgery | Confirmed by her |
| Marta Sánchez | Breast surgery after having her daughter; «no facelift» | Confirmed by her |
| Ana Obregón | Breast augmentation in the 1980s and an implant exchange; denies facial surgery | Reported by Pronto |
| Alaska | Talks openly about her operations without naming them | Confirmed by her |
| Paula Echevarría | Brow micropigmentation; ultrasound lifting treatment | Non-surgical treatments |
| Chenoa | Facial radiofrequency; «not yet» on surgery | Non-surgical treatment |
| Bárbara Rey | Gastric balloon (2020); denies recent touch-ups | Confirmed / denied |
| Aitana | Denies surgery and fillers | Denied |
| Concha Velasco | Said she never had cosmetic surgery | Denied |
| Norma Duval | Denies the speculation after «Mask Singer» | Denied |
| Elsa Pataky | Nose job: she has always denied it | Rumour, denied |
The table is based only on what the stars have said or what the outlet cited in each section reported. Photos alone never prove surgery: lighting, makeup, weight and age change a face a lot.
Sara Montiel and the Facelift: What It Corrects and How Long It Lasts
Yes, and she said so plainly. In 1990, at 62, she told Diez Minutos she had been operated on for «what they call ‘little things’», adding that she would have her neck or eyes done when she needed it. In 1998 she explained that her new look came from laser treatment for freckles and spots rather than a new facelift, while confirming the earlier one: «I did that a few years ago» (Source: Diez Minutos).
The star of El último cuplé (1957) and La violetera (1958), who died in 2013, was a pioneer in talking about it: «I have never hidden these things», she told the magazine in 1998 (Source: Diez Minutos).

What a facelift does. A facelift repositions or removes skin, fat and muscle to treat sagging, deep folds and jowls; it does not stop ageing or fix fine wrinkles or sun spots, and its effect usually lasts about 7 to 10 years. Bruising and swelling last about 2 to 3 weeks, and the risks include haematoma, facial nerve injury and hair loss at the incisions (Source: Cleveland Clinic).
The NHS puts full recovery at 2 to 4 weeks, with bruising for at least two weeks and 6 to 9 months before the final result shows (Source: NHS).
And the laser for spots. Laser skin resurfacing is used for fine lines, sun-damaged skin and liver spots, the same problems Sara Montiel described on her hands and forehead (Source: ASPS).
Belén Rueda and Breast Augmentation: Implant Types and How Long They Last
Belén Rueda has said she had breast surgery but not facial surgery. On the TV show «Planeta Calleja» she said she had her breasts done «at the time», and was firm about her face: she would «never» do it, because facial cosmetic surgery is, in her words, «dangerous, very dangerous» (Source: Trendencias).
Winner of the Goya for best new actress for The Sea Inside (2004) and star of The Orphanage (2007) and the TV series Los Serrano, she is one of the most admired actresses of her generation. Her case shows something common: many women who have breast surgery refuse to touch their face.
What breast augmentation involves. A silicone or saline implant is placed behind the breast tissue or under the chest muscle; shells can be smooth or textured, and shapes round or teardrop. Cleveland Clinic notes that implants typically last about 10 years, that the risk of rupture grows with age, and that more operations may be needed over a lifetime (Source: Cleveland Clinic).
The American Society of Plastic Surgeons (ASPS) says implants are typically exchanged or removed every 10 to 15 years (Source: ASPS).
In Spain, breast implants are medical devices that must carry the CE mark; before surgery the patient must receive written information and sign an informed consent form, and the Spanish regulator AEMPS runs a breast implant registry (Source: AEMPS).

Ana Obregón and Implant Exchange: Capsular Contracture and Rupture
According to Pronto magazine, Ana Obregón had breast surgery while filming Bolero (1984), talked openly about it in 2007, and later went back to theatre to exchange her implants. The magazine adds that she says she has never had facial surgery; of Botox she said: «It terrifies me, just like injections» (Source: Pronto).
The actress and presenter of Ana y los 7 has been one of the most popular faces on Spanish TV for decades, and one of the most discussed for her looks.
Why implants get exchanged. The commonest medical reasons to remove or replace an implant are capsular contracture (the scar tissue around it hardens and may hurt) and rupture of the shell; a ruptured implant should generally be removed (Source: ASPS).
The FDA warns that silicone ruptures are often silent: MRI is the most effective test to detect them, and ultrasound is an alternative (Source: FDA).
Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) is rare: by 30 June 2024 the FDA had 1,380 reported cases and 64 deaths worldwide, and 73% of cases with known details involved a textured implant (Source: FDA).
Removing implants costs $3,979 on average in the US, before anaesthesia and facility fees (Source: ASPS).
Through International Clinics in Turkey, implant removal costs $3,200 (approx. £2,400). Full details in our guide to breast implant removal: cost, surgery and recovery.
Marta Sánchez and Breasts After Pregnancy: Implants or a Breast Lift
Marta Sánchez explained it openly in 2017: after having her daughter Paula her breasts dropped a little and she decided to have surgery, adding that she had not had much more done than her breasts. She also defended women who have surgery: for her, going into theatre «takes a lot of merit» (Source: El Español).
In June 2026, just after turning 60, she insisted she has had «no facelift» and joked about painful beauty treatments: «when it doesn’t hurt, it doesn’t work» (Source: Divinity).
The voice of Olé Olé and of hits such as «Desesperada» and «Soy yo» is still one of Spain’s most searched artists, and her honesty about her breasts is rare among celebrities.
Breasts after pregnancy: implants or a lift? Many women notice emptier or lower breasts after having children. The NHS says implant surgery costs £3,500 to £8,000 in the UK, usually means one to two weeks off work, and that some women need another operation about 10 years later (Source: NHS).
When the main problem is sagging rather than volume, the usual answer is a breast lift, alone or combined with implants.
Through International Clinics, breast augmentation costs $4,400 (approx. £3,300) with Arion implants or $5,200 (approx. £3,900) with Motiva implants; the ASPS US average is $4,875 for the surgeon’s fee alone (Source: ASPS).
A Nose Like Elsa Pataky: What Rhinoplasty Involves and Its Recovery
There is no confirmation: Elsa Pataky has always denied the supposed rhinoplasty, according to Marie Claire, which also lists other rumours about her (buccal fat removal, liposuction) without any source confirming them (Source: Marie Claire).
The same magazine reported in 2023 that the actress «says she has not had surgery», despite all the talk about her changing figure (Source: Marie Claire).
Ironically, her nose has become a model: in 2020 surgeons told ¡HOLA! that photos of Elsa Pataky and Queen Letizia are among those patients bring most often, and Dr Ángel Juárez called her a beauty reference for a whole generation (Source: ¡HOLA!).
The star of Al salir de clase and the Fast & Furious films has long been associated with a healthy, sporty image.
What rhinoplasty involves. It is surgery to change the shape or size of the nose and usually takes 1.5 to 3 hours; a splint stays on for about 7 days, you may need up to 2 weeks off work, and swelling can take up to 6 months to settle completely (Source: NHS).
Risks include a hole in the septum, nosebleeds, changes in the sense of smell and an unsatisfying result; about 15% of people need a revision, and minimal swelling may last up to a year (Source: Cleveland Clinic).
Through International Clinics, rhinoplasty costs $2,500–4,500 (approx. £1,900–3,400), against £4,000–7,000 in the UK. See real results in our rhinoplasty before and after gallery and prices in nose job Turkey cost; if you want to avoid surgery, read about the non-surgical nose job.
Norma Duval and Facial Swelling: How Long It Lasts After Filler or a Facelift
Norma Duval denied any cosmetic work: after her appearance on «Mask Singer» (2020) set off speculation, she wrote on Instagram that her face was swollen and that «none of what has been speculated is true» (Source: COPE).
She blamed the long wait under the Unicorn mask, the heat, the weight of the costume head and her claustrophobia (Source: COPE).
The showgirl who triumphed at the Folies Bergère in Paris is a good example of how one image can feed a rumour. The medical lesson: a swollen face is not proof of surgery. After a filler, swelling usually settles within 24 to 48 hours and can last up to a week; after a facelift, bruising and swelling last weeks (Sources: Cleveland Clinic, Cleveland Clinic).
Bárbara Rey: Gastric Balloon Results and Lip Silicone Removal
Bárbara Rey has disclosed one procedure, though not a cosmetic one: in 2020 she had a gastric balloon fitted to lose weight. She told Diez Minutos she had let herself go and gained «eight or ten kilos», and that she was told the balloon would stay in for three to four months (Source: Diez Minutos).
What an intragastric balloon is. It is placed through the mouth with an endoscope, without incisions, in about 20 to 30 minutes; it takes up space in the stomach so you feel full sooner, and the Orbera model is removed after six months. Average weight loss is about 10% of body weight, nausea is common in the first week, and weight can return if habits do not change (Source: Cleveland Clinic).
On her lips, Diez Minutos reported in 2025 that the actress had injections in the 1990s and, in November 2023, a cheiloplasty to remove non-absorbable silicone. The magazine quotes no statement from her on this, and beauty expert Cuca Miquel warns that such fillers «can create deformities» in the long term (Source: Diez Minutos).
In late 2024, when Lecturas magazine ran photos of her after supposed touch-ups, she replied angrily: «how many lies are they going to invent about me?» (Source: ESdiario).
The actress of La escopeta nacional (1978) and presenter of Palmarés remains a well-known face on Spanish television.
Absorbable vs permanent fillers. Hyaluronic acid lip filler lasts 12 to 18 months and can be dissolved with hyaluronidase; Cleveland Clinic advises against having it in beauty salons or spas, because poor technique can stretch the lips or harden the filler (Source: Cleveland Clinic).
An accidental injection into a blood vessel can block blood flow and, rarely, cause vision loss or stroke (Source: FDA).

Paula Echevarría: Brow Micropigmentation and Focused Ultrasound Lifting
Yes, two non-surgical treatments. In 2018 she shared on Instagram that she had her brows retouched with the «shading» technique, a micropigmentation that gives a softly filled look, saying she was «sooo happy» with the result. According to ¡HOLA!, it takes two sessions four weeks apart and is not advised during pregnancy or breastfeeding (Source: ¡HOLA!).
She also has a yearly ultrasound treatment with a lifting effect, nicknamed «the stapler» in Spain, which her doctor explained to her followers (Source: ¡HOLA!).

The star of Velvet and Gran Reserva is also one of Spain’s most-followed fashion influencers, which is why her treatments attract so much interest.
What focused ultrasound does. It heats deep layers of the skin to trigger new collagen and tighten the face and neck. Some people see improvement straight away, but full results take three to six months; there is usually no downtime, and because ageing continues, touch-ups help maintain the effect (Source: Cleveland Clinic).
Chenoa and Facial Radiofrequency: Sessions, Results and How Long It Lasts
Chenoa says she has not had surgery, at least for now: «I haven’t had surgery… yet», she said in December 2024 on Laura Escanes’s podcast. She also explained that she asked her doctor for a facelift and got radiofrequency instead (Source: ¡HOLA!).
When some viewers claimed in 2022 that she had had surgery, she answered with humour that something had indeed happened to her face: «22 years, for example!». She said she had lost about 10 kilos and asked for respect: «if you don’t like it, don’t look!» (Source: ¡HOLA!).
Since «Operación Triunfo» (2001) and songs such as «Cuando tú vas», Chenoa has gone from contestant to presenter and remains one of the public’s best-loved artists.
What facial radiofrequency is. A device heats the deeper skin layers with electromagnetic waves to stimulate collagen and elastin. Two to six sessions are usual, changes show in two to six months and last one to three years; it works less well on severe sagging and does not replace a surgical facelift (Source: Cleveland Clinic).
Aitana and Appearance Pressure: Body Comments and Body Dysmorphic Disorder
No: Aitana has denied any surgery or injections. In 2020, after an appearance on «El Hormiguero», she wrote that she had «never had any cosmetic surgery or injected anything. It’s makeup» (Source: ¡HOLA!).
In December 2024 the TV show «Fiesta» aired footage of her leaving an aesthetic clinic in Madrid and suggested a lip touch-up; she replied on X that she goes there to have her eyebrows waxed and that she has had «nothing done (nor injected anything) on my face» (Source: Vozpópuli).

The singer of «Teléfono» and the album 11 Razones is one of the most-streamed voices of her generation, and her case shows the pressure young artists face over their looks.
Why this debate matters. Constant comments on someone’s body can affect mental health. Body dysmorphic disorder makes a person spend a lot of time worrying about flaws others barely see, and it is treated with cognitive behavioural therapy and sometimes medication (Source: NHS).
A 2024 meta-analysis estimated it affects 18.6% of plastic surgery patients, and its authors call for screening before surgery (Source: Kaleeny y Janis, 2024).
Alaska and Cosmetic Surgery: Realistic Expectations Before Each Operation
Alaska is one of the few who says it out loud: on «El Hormiguero» in December 2024 she explained that for her «surgery is not about dissatisfaction but about construction», and said that before each operation she writes a letter and prepares a legal document, as a matter of «good sense», not fear (Source: ¡HOLA!).
She has not said which procedures she has had, so any list online is speculation. That same month her documentary series Alaska, revelada premiered on Movistar Plus+ (Source: ¡HOLA!).

The voice of Pegamoides, Dinarama and Fangoria («A quién le importa») has made her image part of her art. Her approach matches what surgeons ask for: the Spanish Society of Plastic Surgery (SECPRE) insists patients need a clear, realistic idea of what surgery can achieve, its limits and its risks, and notes that cosmetic surgery is not covered by Spain’s public health system (Sources: SECPRE, SECPRE).
Concha Velasco and Ageing Naturally: Skin Care Without Surgery
No, by her own account: in 2010, at around 70, she told Pacientes magazine that because she had never had cosmetic surgery she did not have «that strange thing on the mouth», and shared her trick: instead of surgery, when she wanted to look prettier she put on a scarf (Source: revista Pacientes).
The actress of Las chicas de la Cruz Roja and Teresa de Jesús, who died in 2023, championed ageing naturally to the end.
Ageing well without surgery. Dermatologists recommend a broad-spectrum sunscreen of SPF 30 or higher every day, not smoking, gentle cleansing and a daily moisturiser, because the sun is a major cause of premature skin ageing (Source: AAD).
Isabel Pantoja and Malú: Why a Photo Cannot Diagnose Plastic Surgery
Isabel Pantoja: not confirmed. In 2023 an aesthetic doctor consulted by Telecinco claimed, from photos alone, that her main touch-up would be a rhinoplasty; the article includes no statement from the singer (Source: Telecinco).
Malú: not confirmed either. In July 2025 one of her selfies went viral and many said she had had work done; she joked about her «new face… or the old one… or the usual one» and posted another photo without makeup (Source: ¡HOLA!).
A diagnosis from a distance is not a diagnosis: a surgeon needs an examination and a medical history to know what someone has had done.
Spanish Actresses With the Most Surgery: Facelifts, Touch-Ups and Breast Surgery
There is no reliable record, and any «ranking» based on photos is speculation. What can be said is who has been clearest about it: Sara Montiel admitted several touch-ups and a facelift at a time when nobody talked about it, and Alaska discusses her operations openly today (Sources: Diez Minutos, ¡HOLA!).
At the other end, Belén Rueda limits her surgery to her breasts, and Marta Sánchez says she has not had much more done than her breasts (Sources: Trendencias, El Español).
Beautiful Spanish Actresses Without Facial Surgery or Injections
Beauty cannot be measured, but we can say who states she has never had facial surgery: Concha Velasco, Aitana and Chenoa, and Belén Rueda as far as her face is concerned. All of them said so in their own words (Sources: revista Pacientes, ¡HOLA!, ¡HOLA!, Trendencias).

How to Choose a Plastic Surgeon: Official Qualifications and Your Reasons
Start with qualifications. In Spain, cosmetic surgery belongs to the official specialty of Plastic, Reconstructive and Aesthetic Surgery, which requires at least five years of residency after medical school; SECPRE advises always checking that the doctor holds that title (Source: SECPRE).
Then ask yourself why. A good consultation asks what you want to change, why now and what you expect. If the push comes from other people’s criticism, as so many of these artists have faced, talking it through first with a doctor or psychologist is part of good care.
International Clinics is neither a treatment centre nor a hospital broker: it is the link between you and the right treatment. It selected experienced surgeons and well-equipped hospitals, contracted with them against strict standards of medical quality, and prepares a treatment plan for each patient. We are a licensed company with international partners, expanding our hospital contracts to several countries, starting with the UAE.
